Use Clever to access all our technology programs, including Achieve3000 and RazKids.  Use your DISTRICT Google email address (it must end with @carlsbadusd.net) and password to login.  Make sure no other users are logged into Google before accessing Clever.

STEP 1: Log in to your PARENT PORTAL Account and complete the steps in the Summer Registration Instructions

STEP 2: 2018-2019 Residency Verification for ALL students in ALL grades (new and returning)

Thursday, August 23rd 12 pm to 4 pm

 

Required Completed Documents:

 

1. Annual Residency Verification Form (Print from Parent Portal. Copies also available in the office.)

 

2. Two documents from the list below. ORIGINAL and a Copy. The original will be returned to you. **Copies will not be made at school**

  • SDGE bill (must be within 30 days). Entire first page OR a copy of the “Detailed Bill” from the SDG&E website (NOT an SDG&E Letter of Residency).
     

  • Water Bill (must be within 30 days).  Entire first page OR a copy from the City’s website.
     

  • Final Escrow Papers – will require SDGE or Water bill after move in.
     

  • Current Lease Agreement with Management/Landlord’s contact information.
     

  • If the lease has expired and it’s now a month-to-month lease, a letter on company letterhead from the manager/landlord indicating the parents and children are current residents is required.
     
  • Shared Housing – If the bills are in someone else’s name – call the office for more details.

3. Personal Identification of parent/ guardian enrolling.

LA VERIFICACION DE RESIDENCIA ANNUAL es para TODOS los estudiantes de Grados Kinder hasta 5°.  

Guarde toda su factura de luz del mes de Julio y Agosto.

Documentos Requeridos y Completos:

 

1. Annual Residency Verification Form (Forma Residencia Annual de Verificacion). La puede encontrar en la pagina de web de Poinsettia (poinsettiaschool.org) o en la oficina.

 

2. Dos facturas de esta lista: ORIGINAL Y una COPIA. Le regresaremos el original. **La escuela no hara copias**

  • Factura de Luz SDGE (tiene que tener la fecha corriente).
     

  • Si paga por agua, la Factura de Agua (tiene que tener la fecha corriente).
     

  • Si acaba de comprar casa – Los documentos comprobando el compro de casa.(recuerde que necesitara la factura de luz 30 dias despues de cambiarse a la nueva casa)
     

  • Contrato de la Renta – que no este vencido.
     

  • Si el contrato esta vencido, y vive en el apartamento de mes a mes, puede traer una carta del gerente de los apartamentos. La carta tiene que decir quien vive en el apartamento incluyendo los estudiantes.
     
  • Si vive con alguien y la factura de la luz esta al nombre de ellos, hay otra forma que tiene que llenar y mas requeridos. Llame a la oficina para mas detalles.

 

3. Identificacion personal del padre/madre quien va a inscribir al estudiante.

ONLINE enrollment FOR 2018-19 IS OPEN for new incoming students (K-5th)

1st Step:  Immunizations must be up-to-date prior to registration or you cannot pass "Go".  

Special Note: Early check of immunization records is provided at the school's health office prior to registration.

IMMUNIZATION REQUIREMENTS

Immunization Requirements

Parents must provide proof of immunizations (with specific dates and doctor’s signature) for all students when they enroll or transfer. In most cases, proof of immunization will be a yellow California Immunization Record or similar form. The necessary immunizations must include:  

Polio (OPV and/or IPV):     

          Four (4) doses required; three (3) doses are enough if:

a) at least one dose was on or after the 4th birthday for ages 4-6, or

b) at least one dose was on or after 2nd birthday for ages 7-17.    

DTP/DTaP/DT/TD:

Five (5) or more doses.

a) four (4) doses meet requirements for ages 4-6 if at least one was on or after the 4th birthday.

b) three (3) does meet requirements for ages 7-17 years if at least one was on or after the 2nd birthday. If last dose was given before the 2nd birthday, one more (Td) dose is required.

Measles, Mumps, and Rubella (MMR): 

Kindergarden 2 doses Both doses on or after 1st birthday
7th grade 2 doses Both doses on or after 1st birthday
Grades 1-6 1 dose Must be on or after the 1st birthday

Hepatitis B:            

Three (3) doses for kindergarten and 7th grade entry  

  Varicella (Chicken Pox):  Required for kindergarten entry and grades 1-12 for out-of-state students.         

One (1) dose for children under 13 years of age.         

Two (2) doses are needed if child was immunized on or after the 13th birthday.         

Physician-documented varicella (chicken pox) disease history or immunity will also meet the varicella requirement.  

ALL IMMUNIZATION REQUIREMENTS MUST BE MET BEFORE SCHOOL ENTRY. 

Before and After School Care

Before and After School care is provided by the Carlsbad Education Foundation. Please contact them directly at (760) 929-8700. Applications may be obtained through their website https://www.carlsbaded.org/

 

EARLY DISMISSAL E V E R Y THURSDAY at 1:35pm

Early Dismissal for Grades 1st through 5th at 1:35pm. 

TK and Kinder release hours remain the same.
