Immunization Requirements

Parents must provide proof of immunizations (with specific dates and doctor’s signature) for all students when they enroll or transfer. In most cases, proof of immunization will be a yellow California Immunization Record or similar form. The necessary immunizations must include:  

Polio (OPV and/or IPV):     

          Four (4) doses required; three (3) doses are enough if:

a) at least one dose was on or after the 4th birthday for ages 4-6, or

b) at least one dose was on or after 2nd birthday for ages 7-17.    

DTP/DTaP/DT/TD:

Five (5) or more doses.

a) four (4) doses meet requirements for ages 4-6 if at least one was on or after the 4th birthday.

b) three (3) does meet requirements for ages 7-17 years if at least one was on or after the 2nd birthday. If last dose was given before the 2nd birthday, one more (Td) dose is required.

Measles, Mumps, and Rubella (MMR): 

Kindergarden 2 doses Both doses on or after 1st birthday
7th grade 2 doses Both doses on or after 1st birthday
Grades 1-6 1 dose Must be on or after the 1st birthday

Hepatitis B:            

Three (3) doses for kindergarten and 7th grade entry  

  Varicella (Chicken Pox):  Required for kindergarten entry and grades 1-12 for out-of-state students.         

One (1) dose for children under 13 years of age.         

Two (2) doses are needed if child was immunized on or after the 13th birthday.         

Physician-documented varicella (chicken pox) disease history or immunity will also meet the varicella requirement.  

ALL IMMUNIZATION REQUIREMENTS MUST BE MET BEFORE SCHOOL ENTRY.
